Symptom: Approvals tab is empty but I expect decisions

Cause: The queue shows approvals assigned to you or unassigned-but-decidable — items assigned to others, or already decided, do not appear.

Fix: Check the Attention and Activity tabs for context, and confirm with the requester who the assignee is.

Symptom: Approve/reject buttons missing on a row

Cause: Bulk selection only offers snooze/dismiss; decisions require opening the item — and the permission matrix must grant you this approval.

Fix: Open the row detail and decide there; if still missing, you are not an eligible decider.

Symptom: Retry does nothing on a failed execution

Cause: Retry needs tasks edit, and some failures are terminal rather than retryable.

Fix: Confirm tasks edit access, read the ledger error, and escalate terminal failures instead of retrying in a loop.

Symptom: Queue disagrees with module pages

Cause: The hub aggregates with urgency rules and snooze/dismiss state — a dismissed item still exists in its module.

Fix: Check the snoozed section and the source module; restore from the hub if you dismissed by mistake.
